This form is used to allow us to check your credit and to help you understand the fine print before you make Rogers Park Apartments your new home.

Permission to Check Your Credit:
I hereby consent to allow Sweet Home Properties, Inc. and its agent MLC Properties and Management, through their designated agents and their employees, to obtain and verify my credit information, employment and wage information, rental payment history, background information including criminal history for the purpose of determining whether or not to lease an apartment to me. I release from all liability or responsibility all persons requesting or supplying such information. I understand that should I lease an apartment, Sweet Home Properties, Inc. and its agents shall have a continuing right to review my credit information, rental application information, payment history, background information, and occupancy history for account review purposes and for improving application methods. I understand that any negative information can and will result in this application being declined, including any judgements, arrest records and/or collection accounts, and that I will forfeit the application fee.

Acknowledgement of Policies:
The application fee is $50 per applicant and is non-refundable.

Sweet Home Properties, Inc. and its agents fully support Equal Housing Opportunity. We do not discriminate based on age, race, religion, familial status, marital status, sex, sexual orientation or national origin.

The Move-in fee is $500 and is non-refundable. There is no security deposit.

Occupancy limits are two persons per bedroom.

No smoking inside the apartment. There are designated areas on the property for smoking.

Tenant(s) are encouraged to carry Renters Insurance with at least a $100,000 limit for the apartment that names Sweet Home Properties, Inc. and MLC Properties and Management as an additional insured.

Cats and dogs are welcome. Pets must be spayed/neutered, vaccinated, non-aggressive, and older than 6 months. There is a limit of 2 pets per apartment.

ALL THE INFORMATION IS TRUE, ACCURATE AND COMPLETE TO THE BEST OF APPLICANT’S KNOWLEDGE. OWNER RESERVES THE RIGHT TO DISQUALIFY TENANT IF INFORMATION IS NOT AS REPRESENTED. WYOMING APARTMENTS LLC WILL CHECK THE SEX OFFENDER REGISTRY AND RUN A CREDIT AND CRIMINAL HISTORY CHECK ON EACH APPLICANT. APPLICANT HEREBY AUTHORIZES RELEASE OF PERSONAL INFORMATION BY EMPLOYERS, CREDIT BUREAUS, LAW ENFORCEMENT AGENCIES, REFERENCES AND CREDITORS TO SWEET HOME PROPERTIES, INC. AND IT’S AGENTS THROUGH THIS RELEASE FORM OR A PHOTOCOPY OR EMAIL SCAN THEREOF. CLAIMS FOR REWARDS FROM MULTIPLE REFERRAL SOURCES WILL RESULT IN APPLICANT BEING CHARGED ROGERS PARK APARTMENTS’ ADVERTISING COST FOR THE REFERRAL.

LANDLORD, CREDIT, CRIMINAL AND SEX OFFENDER CONSENT

I hereby give consent to Management of the above-named apartment community to obtain reports and to access any records pertaining to me, which may be on file with any: Landlord; City, State or Federal Court; Management Agency; Local, State or Federal Agency; Credit Agency; State or Local Repository; Law Enforcement Agency; National, State or Local Sexual Offender Registry. I understand the investigation will include information from law enforcement agencies, credit reporting agencies, and other documents of public records, and these reports will be used in making decisions about my potential tenancy. I hereby authorize any agency contacted to furnish any and all information required. This releases the aforesaid parties from any liability and responsibility for providing the above information at any time. I further understand that this report will not be used in violation of any Federal or State Equal Opportunity Law or Regulation, and that, if any adverse action is to be taken based on the Consumer Report, a summary of my rights under the Fair Credit Reporting Act will be provided to me.

PENALTIES FOR MISUSING THIS CONSENT: Title 18, Section 1001 of the U.S. Code states that a person is guilty of a felony for knowingly and willingly making false or fraudulent statements to any department of the United States Government. HUD and any owner (or any employee of HUD or the owner) may be subject to penalties for unauthorized disclosures or improper uses of information collected based on the consent form. Use of the information collected based on this verification form is restricted to the purposes cited above. Any person who knowingly or willingly requests, obtains, or discloses any information under false pretenses concerning an applicant or participant may be subject to a misdemeanor and fined not more than $5,000. Any applicant or participant affected by negligent disclosure of information may bring civil action for damages and seek other relief, as may be appropriate, against the officer or employee of HUD or the owner responsible for the unauthorized disclosure or improper use. Penalty provisions for misusing the social security number are contained in the Social Security Act at 42 USC 208 a(6)(7) and (8). Violations of these provisions are cited as violations of 42 USC 408 a(6)(7) and (8).
